Required Forms
HTHNC 2022-2023 Internship Handbook
Students, Parents, and Mentors must read, sign, scan and then upload the HTHNC Internship Handbook to this Google Form. This will then be reviewed by Tom Gaines (Internship Coordinator).
1) Read and Sign Internship Mentor Commitment Form (pgs. 8-9 of Handbook)
2) Read and Sign Parent/Guardian Internship Consent Form (pgs. 10-12 of Handbook)
3) Once these Forms in the Handbook are signed and uploaded to this Google Form, and approved by the Internship Coordinator, your Internship is official.

Timesheets
Students will be given a timesheet that will need to be completed daily. Please have your mentor sign the timesheet at the end of each week to confirm your attendance. This timesheet must be submitted to your teacher weekly on each Friday, and at the conclusion of internship.
